THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
How We May Use and Share Your Information
CAVU Psychiatry may use or disclose your protected health information (PHI) as permitted or required by law.
Treatment — We may use and share your information with healthcare professionals involved in your care. For example, we may share medication information with your primary care provider or pharmacy to coordinate your treatment.
Payment — We may use and share information to bill your health plan, obtain payment, determine benefits, or obtain authorization for services.
Healthcare Operations — We may use and share information to operate CAVU Psychiatry, including billing, quality improvement, compliance, credentialing, and other healthcare operations.
We may also use or disclose information when permitted or required by law, including for public health and safety, reporting suspected abuse or neglect, health oversight, workers' compensation, certain law-enforcement purposes, judicial or administrative proceedings, and preventing or reducing a serious and imminent threat to health or safety.
For uses or disclosures not otherwise permitted or required by law, we will obtain your written authorization. You may revoke an authorization in writing, except to the extent we have already acted in reliance on it.
Your Choices
When permitted by law, you may tell us whether you want information shared with family members, caregivers, or others involved in your care.
Certain uses and disclosures, including most uses of separately maintained HIPAA-defined psychotherapy notes, marketing, and the sale of PHI, require written authorization when applicable.
Substance Use Disorder Records
Certain substance use disorder records may receive additional protections under 42 CFR Part 2. When these protections apply, CAVU Psychiatry will use and disclose such records in accordance with federal law.
Part 2 records generally may not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you without your written consent or a qualifying court order as required by law.
Your Rights
You have the right to:
Obtain an electronic or paper copy of your health information.
Ask us to correct information you believe is incorrect or incomplete.
Request confidential communications.
Ask us to limit certain uses or disclosures of your information.
Request that information about services you paid for in full not be disclosed to your health plan for payment or healthcare operations when required by law.
Request an accounting of certain disclosures of your health information.
Obtain a copy of this Notice at any time.
Have a legally authorized representative exercise your rights when permitted by law.
File a complaint if you believe your privacy rights have been violated.
Our Responsibilities
CAVU Psychiatry is required by law to:
Maintain the privacy and security of your protected health information.
Notify you as required by law if a breach occurs that may have compromised your information.
Follow the privacy practices described in the Notice currently in effect.
Provide you with a copy of this Notice upon request.
We may change this Notice. Any changes may apply to information we already maintain as well as information created in the future.
